Cancer two-week wait referrals
Two week-wait appointments are not currently offered via Choose and Book. If you are referring a patient to a cancer two-week wait clinic, please use the "Pan Birmingham Cancer Network Urgent Referral Forms". Fax the completed referral form to the cancer fax gateway on:
