Rights of access to information

Freedom of Information Act 2000

In addition to accessing the information identified in the publication scheme, you are entitled to request information which we hold and have recorded in any form. The right to access this information is subject to certain exemptions listed in the Freedom of Information Act. Where we decide to withhold some or all of the information requested, we will inform you of the reasons for this.  

Before making a formal request to the Trust, please check our website to make sure that the information is not already available. Our publications scheme has most of the information routinely asked for by members of the public. If the information you require is not held on our website, you can request it by post or email:

Data Protection Act 1998

Under the Data Protection Act 1998, you are also entitled to access your clinical records or any other personal information held about you. Please write to:

You have the right to see information held about you, except in the rare event where a health professional believes that this would cause serious physical or mental harm to you or anybody else. You would not normally have the right to access personal information about someone else.

Please note that we will request proof of your identity and may charge a fee. Under the Data Protection Act we are obliged to comply with your request within 40 working days of receipt of the request or from the receipt of the information necessary to enable us to comply with your request, whichever is later.
