Patients' handover

The following information must be communicated with the ward to where the patient is being transferred:
- Introduce yourself, and clearly state that you are calling to arrange a transfer of a patient to the ward
- Identify the patient (name, hospital number)
- Summarise the case,withthepatient’s:
- age and gender
- the date and time of their admission to the hospital
- diagnosis during their admission
- COVID-19 result (positive, negative, pending)
- Risk of exposure to COVID-19 (at community level, on the current/previous wards)
- list of comorbidities
- need for any IV medications and, if so, when is the next dose planned for
- most recent set of observations with time and date
- laboratory results that are abnormal/pending
- Document your conversation with the ward staff (and their name) on the patient’s notes
- Do not transfer patients before discussing them with a nurse on the ward
