Patients' handover

The following information must be communicated with the ward to where the patient is being transferred:

  • Introduce yourself, and clearly state that you are calling to arrange a transfer of a patient to the ward
  • Identify the patient (name, hospital number)
  • Summarise the case,withthepatient’s:
    • age and gender
    • the date and time of their admission to the hospital
    • diagnosis during their admission
    • COVID-19 result (positive, negative, pending)
    • Risk of exposure to COVID-19 (at community level, on the current/previous wards)
    • list of comorbidities
    • need for any IV medications and, if so, when is the next dose planned for
    • most recent set of observations with time and date
    • laboratory results that are abnormal/pending
  • Document your conversation with the ward staff (and their name) on the patient’s notes
  • Do not transfer patients before discussing them with a nurse on the ward
