Reason for leaving

It is important for managers completing a leaver's termination form to select the correct reason why the member of staff is leaving.

Understanding why staff leave the Trust helps to identify any patterns or trends. This enables the Trust to make changes to the working environment for the benefit of all staff.

The following guidance outlines the key reasons for staff leaving and details why each option should be selected when completing a termination form. If the exact reason the member of staff is leaving is not listed, please select the option that is the closest match.

End of fixed term contract

Reason for leavingIncludes leaving due to
Completing training scheme Completing an apprenticeship or other training contract with the Trust (e.g. doctors in training)
End of work requirement Completing a designated work project

Voluntary resignation

Reason for leavingIncludes leaving due to
Adult dependents * Caring for adult dependents
Child dependents * Caring for children
Health * Personal ill health that results in the staff member deciding to leave their role
Better reward package Moving to a similar role with better rewards (i.e. higher salary, pension or annual leave)
Incompatible working relationships Breakdown of working relationships between a manager or colleagues
Lack of opportunities Shortage of opportunities for development or promotion within their area of work
Promotion Moving to a higher band or more senior role
Relocation Moving house to another part of the country or leaving the UK
Undertaking further education or training Starting a university/college course or training programme outside of the Trust
Work/life balance * Work no longer being compatible with their personal life and other commitments
Retirement age The employee reaching pensionable age and choosing to retire
Other/not known Do not use this option as it does not provide the required information to make UHB a better workplace

* Managers should ensure that flexible working has been offered to the member of staff if this is their reason for leaving the Trust.

Retirement

Voluntary early – actuarial reduction Retiring early which affects pension benefits
Voluntary early – no actuarial reduction Retiring early which does not affect pension benefits
Flexi retirement The employee retiring but returning to the Trust

Dismissal

Dismissal options should only be selected after consultation with HR.

Reason for leavingIncludes leaving due to
Conduct Dismissal under the disciplinary procedure
Capability Dismissal under the sickness absence or performance procedures
Other substantial reason Dismissal as a result of a formal process

Redundancy

Redundancy options should only be selected after consultation with HR.

Reason for leavingIncludes leaving due to
Compulsory Employee being made redundant
Voluntary Employee choosing to take voluntary redundancy

Other

Reason for leavingIncludes leaving due to
Bank staff Minimum work requirement not fulfilled (UHB+ use only)
Honorary contract ending An honorary contract has expired or the individual has left
Death in service An employee sadly passing away whilst still in employment at UHB
