Questions

Which medicine formulary/formularies the University Hospitals Birmingham NHS Foundation Trust adheres to? The Heartlands, Good Hope and Solihull Hospitals adhere to the Birmingham and Solihull Integrated Medicines Optimisation Committee formulary where-as the Queen Elizabeth Hospital adhere to the Trust's own formulary. Is this still correct, or do all the hospitals of the entire Trust adhere to a single formulary now?

The link you have provided to the online version of the University Hospitals Birmingham (UHB) Formulary is correct and covers all UHB sites, except when referring items for GPs to prescribe. In these instances all UHB sites align to the Birmingham and Solihull Integrated Medicines Optimisation Committee formulary.
