This privacy notice is here to help you understand how we look after your personal information. We want you to know what we collect, why we collect it, how we keep it safe, and who we might share it with.
A privacy notice is a bit like a rulebook that explains how your information is used. If anything doesn’t make sense, you can always ask a parent, carer or an adult you trust to at our hospital to help explain.
What is personal information?
Personal information is anything that can tell someone who you are. Some examples are:

- your address
- your birthday
- your photo
- your email
- your hospital records
This is information about you that we will keep safe.
What happens at the Hospital?
At University Hospitals Birmingham NHS Foundation Trust (UHB), we are trusted like your parents or teachers and you can share your personal information with us.
When you visit the hospital, all staff members will be wearing official UHB badges. These badges help you identify people who work at our hospital like doctors and nurses.
Our staff may ask for your personal information, so you can feel confident that it’s safe to share your personal information with them. That’s because:
They need it to help you get better.
They may need to contact you if they want to check on you again later.
Sometimes we collect this information:
- on paper
- by phone
- on the internet
- from you, your parents/carer or hospital staff
What do we do with you information?
We might keep your information on paper or electronically, like on a computer. All of this information together is called your ‘health record’. You might also hear it called your ‘notes’ by our doctors and nurses.
Anyone taking care of you at our hospital, can see what information has been collected. We use your health records to make the right choices about your care. We may share or use your information to:
- help you and your doctors make choices about your health
- make sure you care is safe
- work with others to give you the right treatment
- make sure everyone gets the best care
- ask questions and understand how we can give the best care
- help with any worries or complaints
Who might we share your information with?
We keep it safe and only share it when it’s really important, like with:
- other doctors and nurses
- ambulance staff
- your parents
- your GP
- your school
- the Police (if needed)
Your rights
You (and your parent or carer) have rights over your information. ‘Rights’ are rules that say how we must look after your information and what you can ask us to do with it. You can:
- ask to see the information we have about you
- ask us to correct it if it’s wrong
- ask questions about how we use it
- say no to it being used for things like research or planning (unless the law says we must)

Our data protection officer has an important job. Their job is to make sure your information is kept safe and used properly. You can contact them if you have questions or concerns.

If you or your parent/carer are worried that your information is not being looked after properly, you should contact our data protection officer.
If you are still worried or feeling unhappy, you can contact the Information Commissioner’s Office (ICO). The ICO makes sure everyone keeps your information safe and follows the rules. You can visit their website and get in touch by telephone or via email.
