Solihull Stroke and Neuro Rehab Service

Who we are? 

We are a therapy driven team supporting our Early Supported Discharge (ESD) patients on discharge from hospital to home. These discharges are co-ordinated by a team of doctors, nurses and therapists.   

Our team includes: 

  • Physiotherapists 
  • Occupational therapists 
  • Therapy support workers  
  • Speech and Language therapists 

What we do? 

The Solihull ESD team is made up of stroke and neuro-specialist staff to deliver a 6-day service between the hours of 8:00 and 16:30. We provide early neurological rehabilitation for any Solihull resident who has suffered a stroke for a period of up to six weeks.  

Referrals are accepted from any trust if they meet the criteria of residing within the catchment area and requiring ESD therapy input. 

Delivering Your Care 

The therapy services provided work towards achieving agreed patient goals to enhance functional progress. The team will meet patients, their families (and dogs) in their own home. A full neurological assessment will be completed, and a comprehensive treatment plan will be established to work towards meeting agreed goals.  

This is achieved through: 

  • Motor learning practice 
  • Activities of daily living practice (eg washing and dressing/meal preperation) 
  • Cognitive therapy 
  • Communication  
  • Swallowing support 
  • Emotional and caregiver support 

What happens after six weeks?  

If goals are ongoing and progress is being made, we have the option to refer on to our new Solihull Community Stroke team who can continue to help guide your therapy.
