UHB health talk on community services transformation in Solihull

Join us on Wednesday 23 September to learn about the transformation of community services in Solihull.
The talk will take place between 15:00-16:00 in the Solihull Hospital Lecture Theatre which is in the Education Centre on the first floor. You can also join via MS Teams. The talk is aimed at UHB members, staff and the local community.
The programme of transformation has been developed by the Solihull Hospital and Community Services Senior Team, with partners from primary care, public health, social care and the voluntary sector.
It has been approved by NHS Birmingham and Solihull Integrated Care Board and will see an investment of over £5million.
The programme will shift care from hospital-centric models towards proactive, preventative, and integrated community-based pathways, focused on frailty, long-term conditions, end-of-life care, respiratory disease, and improved patient flow.
