The Solihull Community Diabetes service provides specialised short-term intervention for adults with Type 1 or Type 2 diabetes, enabling and empowering them to achieve their optimum health outcomes.
The service is run by Consultant Diabetologists, Diabetes Specialist Nurses and Specialist Diabetes Dietitians and provides a range of patient education programmes.
Planning for your appointment:
Waiting times in clinics can vary and planning ahead can make your visits easier.
Here are some suggestions:
- If you monitor your blood glucose levels at home, please bring your readings or your blood glucose meter to your clinic appointment.
- Write a list of any questions you want to ask and take it with you.
- You may wish to bring a list of your current medications with you.
- You are welcome to bring someone with you.
- You may want to bring a snack and a drink with you.
What we do
The team provides specialist treatment, advice and training in:
- Consultant led diabetes clinics
- Dietetic advice - one to one appointments and group sessions
- Diabetes care that includes:
- Insulin and GLP-1 analogues initiation, titration and self-management
- assessment and titration of oral hypoglycaemic agents
- flash/continuous glucose monitoring initiation via online webinars or one to one appointment for patients who meet the current criteria
- blood glucose monitoring training
- Facilitating hospital discharges
- Diabetes home care:
- Diabetes Specialist Nurses and Dieticians support at home for house bound patients
- Diabetes Specialist Nurse and Dieticians support at Care Homes and Nursing Homes – for individual patient care and education of care home staff
- GP/Practice Nurse support through:
- telephone advice/email advice
- in-practice teaching

Diabetes Education Programmes
DAFNE (Type 1)
Blended learning education programme facilitated by accredited DAFNE educator. Content includes:
- Blood glucose checks, insulin injections, ketones checks and all aspects of carbohydrate counting
- Insulin and insulin adjustment: blood glucose targets and diaries, corrections, stepwise approach, increasing and decreasing insulin doses, insulin and snacks
- Low blood glucose, causes and treatment (why is optimal control important, eating out, alcohol, physical activity, minor hypos and driving, severe hypos)
- High blood glucose, causes and treatment (ketones, illness management and sick day rules, making plans)
Programme aim
Aims to help adults with type 1 diabetes lead as normal life as possible, whilst also maintaining blood glucose levels within healthy targets to reduce the risk of long-term diabetes complications.
Duration and mode of delivery
The course is delivered online by Specialist Diabetes Nurse and Specialist Diabetes Dietitian via Microsoft Teams video calls (3 - 3.5hrs long) one day a week over 6 weeks and self-directed online learning (3 - 6hrs).
VICTOR (Type 2)
The aim of VICTOR is to help adults living with type 2 diabetes to manage their diabetes by developing skills in insulin dose adjustment and improving insulin sensitivity through lifestyle changes, both of which will contribute to reducing short and long-term diabetes related health problems.
The focus is on improving insulin sensitivity using a ‘Smart Seven’ approach, to reduce post prandial glucose levels through changes to diet and physical activity.
Programme aim
To support people living with type 2 diabetes to manage their diabetes by developing skills in insulin dose adjustment
Duration and mode of delivery
The course is delivered online by Specialist Diabetes Nurse and Specialist Diabetes Dietitian as video calls (3 - 3.5hrs long) one day a week over 6 weeks alongside weekly self-directed online learning (3 - 6hrs).
Insulin Titration Education
The focus is on increasing participant’s knowledge of the mode of action of different insulins, and factors impacting blood glucose variability such as dietary patterns and activity levels on glucose variability.
Introduces self-management skills to follow a structured approach for safely adjusting insulin based on glucose trends and individualised blood glucose targets.
Includes a practical application of learning through case studies to apply skills following written guidance.
Aim
To support people living with type 2 diabetes to develop self-management skills to safely and confidently adjust their insulin.
Duration and mode of delivery
Individual clinical appointments supported by a Diabetes Specialist Nurse.
Carbohydrate Awareness Workshop
The focus is on increasing participant’s knowledge of sources of carbohydrate in the diet, and the impact of different types of carbohydrate or dietary patterns have on blood glucose variability.
Introduces self-management skills to estimate total carbohydrates; visual estimation, carbohydrate reference tables and weighing portions.
Includes a practical application of learning through label reading, calculating recipes and keeping a food and blood glucose diary.
Programme aim
To support people living with type 2 diabetes to identify and safely modify their carbohydrate intake line with evidence-based nutrition guidelines.
Duration and mode of delivery
A 3 hour in person group workshop delivered by a Diabetes Specialist Dietitian. Includes 60 minutes telephone contact with Specialist Diabetes Dietitian to set and review goals.

The Solihull Community Diabetes Team provides care for patients registered with a Solihull GP. To submit a referral please use the form in the downloads section below.
If you are registered with a Birmingham GP, please ask your healthcare professional to refer you through the Birmingham Community Healthcare referral process.
