The Paediatric Ambulatory Treatment of Children at Home (PATCH) team provides short-term care to appropriate children to facilitate early discharge. We also provide support to long term nephrology and endocrinology patients and families. This means that the care needed continues to be provided at home through a home visit schedule.
With over 100 years combined experience we are proud to be implementing care and support to our children and young people and families across a large demographic area. We pride ourselves in delivering excellent care that prevents many children from having prolonged stays within a hospital setting, often preventing admission completely.
There are certain safety criteria that apply in order for PATCH to be able to provide care at home, therefore not every child will be eligible for this service. Your child’s safety and wellbeing are our priority, and we will discuss this with you if your child’s care is able to be supported by this service. If you would prefer your child to stay in hospital for their care this will remain an option for you.
