Clinical guidance

Bile acids are produced in the liver from cholesterol and stored in the gallbladder before being released into the small intestine, where they aid the digestion and absorption of dietary fats. The total serum bile acid concentration reflects synthetic, excretory and re-absorptive functions of the liver. Bile acid measurement is useful in the diagnosis of disorders of the hepatobiliary system. Bile acids are increased in patients with acute hepatitis, chronic hepatitis, liver sclerosis and liver cancer. Measurement of serum bile acids is primarily used in the investigation of intrahepatic cholestasis of pregnancy (ICP). In ICP, serum bile acid concentrations are typically increased and may become abnormal before changes are seen in routine liver function tests, making bile acid measurement a sensitive marker for the condition.
