A referral form is accepted from a health care professional (e.g. GP, Practice Nurse or Community Nurse), which clearly identifies the reason you are being referred.
If you feel you would benefit from the service and would like to know more, ask your GP or Practice Nurse/Specialist Nurse to refer you.
If you would like to attend the X-PERT Insulin program, you do not need a referral from your GP, instead contact the service and make a request for further information that can be sent to you.
