Request access to health records

You have a legal right to access personal information held about you; this is called a subject access request under data protection law. In some cases you have the right to access information about a deceased individual.

To access your personal information or make a request on behalf of someone else, please write to the Access to Health Records department and include:

  • full contact details
  • proof of identity
  • description of what data you would like

We have a legal duty to respond within 40 days once we receive any fee (if applicable) and the required details to identify you and the information you need. We will write to you if we need further details to help us find the information you require or identify you. Your request will be dealt with once we have all the necessary information and any applicable fee.
