You have the right to request access to personal information held about you. This is known as a Subject Access Request under data protection legislation.
You may also be able to request information on behalf of another person where you have the appropriate authority. In certain circumstances, access to the health records of a deceased individual may be available under the Access to Health Records Act 1990.
There is normally no charge for making a request.
Before you apply
We may require proof of identity and, where applicable, evidence of authority or entitlement before we can process your request.
Subject Access Requests are normally responded to within one month of receiving a valid request and any information required to confirm identity and entitlement. In some circumstances, this timeframe may be extended in accordance with data protection legislation.
