About the service

Neighbourhood Health is based on a simple principle: good health is about much more than healthcare alone. By working more closely together, partners will provide more personalised, preventative and coordinated support, helping people access the right care, in the right place, at the right time. The approach focuses on prevention, early intervention and coordinated support. 

A key part of the programme is the development of Neighbourhood Health Centres, bringing together a range of services including general practice, community services, diagnostics, outpatient care, social care, housing support and urgent care. General practice will play a central role, supported by Integrated Neighbourhood Teams (INTs), which bring health, care, community and voluntary sector organisations together to support people with complex needs.

Good health is influenced by many aspects of daily life, including housing, employment, social connections and access to local support. By working together and sharing expertise, organisations can better understand local needs, reduce health inequalities and make sure people receive the right support at the right time. This includes listening to communities whose voices may not always be heard and ensuring services are shaped around what matters most to local people. 

Neighbourhood Health Centres will bring many of these services together in local communities, making care more convenient, connected and easier to access. 

Neighbourhood Health supports the Government's vision to move care from hospitals into communities, focus more on prevention and make better use of digital technology. By working together, we can help more people live healthier, happier and more independent lives, while reducing health inequalities and improving care for everyone.

Neighbourhood Health has evolved from the Birmingham and Solihull’s Community Care Collaborative (CCC). The CCC, led by Birmingham Community Healthcare (NHS) Foundation Trust (BCHC) worked with health and care partners to deliver more joined-up, neighbourhood-based care and support people to live well for longer in their own homes. BCHC has become a core delivery and strategic partner of Neighbourhood Health. The latest Neighbourhood Health guidance can be found on Department of Health & Social Care website.
