About us
We focus on assisting our patients to recover from major heart or lung procedures and adapt to their new functional abilities within their everyday life.
Within our role we deliver comprehensive functional and cognitive assessments to support recovery after surgery or injury, this enables us to effectively plan for discharge, including the provision of appropriate equipment and the identification of requirements for ongoing care packages.

What to expect
We will talk with you about your daily life, home environment and support network to understand how your illness may have affected your ability to manage at home.
When you are medically well enough, we will assess how you manage everyday activities such as washing, dressing and moving around. This helps us identify any equipment, adaptations or support you may need after discharge.
We may carry out assessments of your memory, thinking and problem-solving skills using observation and standardised assessment tools. This helps us identify any changes that could affect your safety and independence at home and determine whether additional support is needed.

How to be referred
We screen and triage the in-patient wards daily to establish which patients require input from our team. Referrals can also be submitted by ward-based teams via an online system. They are initiated when there are concerns regarding a patient’s physical or cognitive ability to manage activities of daily living.
We currently do not have an out-patient service.
