About us

As a surgical team, we work with patients experiencing both acute and long-term health conditions, supporting them through preparation for and recovery from transplants and general surgical procedures. Our role includes delivering complex rehabilitation to facilitate safe discharge or transfer to specialist rehabilitation centres. 

We carry out comprehensive assessments of patients’ physical and cognitive needs using functional and standardised assessment tools. This enables us to effectively plan for discharge, including the provision of appropriate equipment and the identification of requirements for ongoing care packages.

What to expect

  • Gathering comprehensive social history to identify changes following illness and to assess the patient’s home environment 
  • Conducting physical assessments of functional ability (e.g. washing and dressing) to determine equipment and care needs once the patient is medically stable for discharge 
  • Completing cognitive assessments, using both functional observation and standardised tools, to identify any changes or impairments that may impact a patient’s ability to function safely and independently at home, and to determine the need for additional support

How to be referred

Referrals are submitted by ward-based teams via PICS, an online system. They are initiated when there are concerns regarding a patient’s physical or cognitive ability to manage activities of daily living.
