Vulnerabilities

Do you or your loved one who is having surgery have a learning disability, autism or dementia? 

In order to ensure that all of our patients receive the same high standards of care, we have a team that can support those patients who may be vulnerable and attend our hospitals. 

The vulnerabilities team is made up of a group of nurse specialists who support adults, young people and children with: 

  • learning disabilities 
  • autism  
  • dementia 
  • mental healthcare needs 
  • hearing and vision impairments 

If you think you need support whilst you are in hospital, please contact the team on the advice line, Monday to Friday, 08:00 - 16:00.

During your pre-assessment appointment, you can ask for a passport that will help you when you need to come or stay in hospital.

  • Learning disabilities/autism – hospital Passport 
  • Dementia – all about me 
  • Hearing and vision – communication passport 
  • Mental Health – Wellbeing Passport 

Whilst in hospital you may require reasonable adjustments.

Reasonable adjustments are a legal requirement under the Equality Act 2010. All organisations, including health and social care, should be accessible to those with a disability or a long-term health condition. 

reasonable adjustments can include: 

  • allowing extra time for an appointment 
  • inviting a relative/carer to come along to your appointment 
  • using alternative communication methods 
  • adapting the environment (different lighting or being given an appointment at a quieter time of the day)
